Lower the Dose: Why the System Gap Is the Missing Link in Diabetes Care

If you or someone you love is living with diabetes, you probably know how complicated it can feel: multiple doctors, tests, medications, appointments, and still, outcomes are often not where they should be. On the latest episode of The Diabetes Blueprint, Dr. Rangi unpacks one of the most critical and overlooked barriers to diabetes success: the system gap.

The Shocking Disconnect in Diabetes Care

Here’s a statistic that should shake us all: 94% of adults with diabetes in the U.S. are connected to healthcare, yet only 23% achieve their key health targets blood sugar, blood pressure, and cholesterol. Dr. Rangi explains at 01:05 that the system gap connects all the other gaps in diabetes care: diagnosis, remission, protection, monitoring, and education. “Our bodies work as one connected system. But our healthcare doesn’t,” she says. Instead of treating the whole person, the system shuffles patients from one organ specialist to another, rarely connecting the dots.

Real Lives, Real Consequences

One of the most moving parts of the episode comes as Dr. Rangi recounts the story of a patient who lost his leg after years in the healthcare system with “dangerously high” blood sugars (06:10). Despite being ‘in the system’ seeing doctors, filling prescriptions, attending appointments no one coordinated his overall care. No one intensified his treatment plan. By the time his complications were recognized, it was too late. “The system does not fail at just one point,” Dr. Rangi says at 05:57. “It fails in the connections between points.”

And he’s not alone. Whether it’s misdiagnosed diabetes types or preventable strokes, these tragic outcomes are repeating daily because no one’s acting as the “quarterback” for a patient’s whole game plan.

Becoming Your Own Quarterback

So what’s a patient supposed to do? Layla and Dr. Rangi drive home the need for patients to become their own quarterback (11:33), not their own doctor. You don’t need to self-treat, but you do need to make sure someone’s coordinating your care.

Dr. Rangi suggests starting with one powerful question: Who is coordinating my care across ALL my conditions? (09:01). This single question opens up the visibility needed for team-based, holistic care.

The Six Questions Every Patient Should Ask

To make this practical, Dr. Rangi offers six gaps and six essential questions you can bring to your doctor (13:05, 24:08):

  1. Diagnosis Gap: Are you sure I have Type 2 diabetes?

  2. Remission Gap: Where am I on the timeline can I still achieve remission?

  3. Protection Gap: Am I on any organ-protective medications? If not, should I be?

  4. Monitoring Gap: Can I try a continuous glucose monitor?

  5. Education Gap: Has anyone referred me to diabetes education?

  6. System Gap: Who is coordinating my care across all my conditions?

You don’t need a medical degree to ask these just your voice and willingness to speak up (15:48, 20:06).

Why This Matters: Taking Back Control

Dr. Rangi is clear: while there are dozens of gaps in the healthcare system that require big institutions or policymakers to fix, these six are in our hands as patients right now (19:03). You don’t have to wait for Congress, insurance companies, or new technologies.

She urges patients to walk into appointments not just as recipients of care, but as advocates for their health: “Please show me the plan. Please show me my timeline. Where am I, and can I reverse my diabetes?” (20:13)
